a. Report Card (for Regular Fist Year Student)
b. Honorable Dismissal & Informative copy of Transcript of Records (for Transferees)
c.Certificate of Good Moral Character (from Principal/Class Adviser/Head Student Personnel Services)
d. Police Clearance
e. Photocopy of Marriage Contract (for Married Applicants)
Note: Original copies of credentials are to be submitted to the Registar. Records may be withdrawn if applicant does not qualify or decides not to enroll. SCHEDULE OF ENTRANCE EXAMINATIONS

Venue: Guidance Center (8:30 a.m.-12nn)
Note: Applicants are required to bring pencil and 1 long paper during the exam. Tests will be administered in the morning only. Examinations will start on time. Latecomers will be rescheduled.
